Graded Care Profile -Trainers Needed

For a number of years, Health Partners have facilitated the multi-agency training on the Graded Care Profile tool. Hertfordshire Community NHS Trust had a pool of 3 facilitators who covered this training but due to staffing changes, following retirement/maternity leave, HCT can no longer solely facilitate this training. They would welcome partners from another agencies to help co-facilitate the GCP training, as they feel they would benefit from the knowledge/expertise of other agencies.
